- CFS 1–3: not frail — use standard risk assessment
- CFS 4–5: mild frailty — consider enhanced recovery, geriatric input pre-op
- CFS 6–7: moderate-severe frailty — high surgical risk, MDT discussion essential
- CFS 8–9: very severe/terminal — consider goals of care, palliative input
- Used in COVID-19 critical care triage (NICE, NHS England)
