Prior stroke, TIA, or thromboembolism+2 Congestive heart failure / LV dysfunction+1 Hypertension+1 Diabetes mellitus+1 Vascular disease (MI, peripheral artery disease, or aortic plaque)+1

ESC 2020 Guidance

Male ≥2 or female ≥3: anticoagulate (DOAC preferred)
Male 1 (non-sex): consider anticoagulation based on individual assessment
Male 0 or female 1 (sex only): no anticoagulation needed
Always assess bleeding risk with HAS-BLED
