Use IBW if patient is obese:
IBW(male) = 50 + 2.3 × (height in inches − 60)
IBW(female) = 45.5 + 2.3 × (height in inches − 60)
Drug Dosing Thresholds
  • >60 mL/min — Normal doses
  • 30–60 — Dose-reduce: LMWH, DOACs, metformin, aciclovir, many antibiotics
  • 15–30 — Significant reductions; avoid metformin, methotrexate
  • <15 — Most renally-cleared drugs contraindicated. Consult renal pharmacist.
