Drug Dosing Thresholds
- >60 mL/min — Normal doses
- 30–60 — Dose-reduce: LMWH, DOACs, metformin, aciclovir, many antibiotics
- 15–30 — Significant reductions; avoid metformin, methotrexate
- <15 — Most renally-cleared drugs contraindicated. Consult renal pharmacist.
