Interpretation
Score <4: Low risk (~0.3%) — mechanical prophylaxis only
Score ≥4: High risk (~11%) → pharmacological thromboprophylaxis
(unless bleeding risk outweighs benefit)
When to use
- All medical inpatients on admission — mandatory VTE risk assessment (NICE QS3)
- Reassess if clinical status changes during admission
- Balance against IMPROVE Bleeding Risk Score before prescribing LMWH
- Not validated for surgical patients — use Caprini/RCOG/NICE surgical guidelines
