When to use
- Sepsis with lactate ≥2 mmol/L on presentation — recheck at 2 hours
- Monitoring response to fluid resuscitation and vasopressors
- ICU and ED acute sepsis management
- Guidance on escalation of care
Formula
Total clearance (%) = ((Initial − Repeat) ÷ Initial) × 100
Clearance per hour = Total clearance ÷ hours between measurements
Target: ≥10% per hour (or ≥20% total at 2h)
